Amherst Dr., Tyler, Tx  75701, with a check or money order payable to Billie Smith .  Late arrivals or cancellations will be given a ticket to atttend  the next scheduled class. There will be NO REFUNDS , unless the class is canceled by the Provider.  The courses are valid for two years or until the license is renewed.  This form can also be sent in with any sharpening order.
